Making things difficult for Jim (and you) are a throng of enemies including chickens, freaks and monsters that were hatched in Jim's own mind. The adventure isn't a cakewalk and, in addition to the collection of udders and marbles, you have plenty of things to deal with inside Earthworm Jim's head such as many side quests required to complete the levels. Each shows you a wacky side of Jim as take up the challenge of retrieving the "Golden Udders." And, since he has lost his marbles for the time being, you must find those as well! However, the circumstances of his return aren't happy as he is faced with one of his greatest challenges ever - a cow-induced blow to the head! Jim is forced to fight his own subconscious and conquer four main areas of his brain: memory, fear, fantasy and happiness. Same worm, different appearance!
